Job Description:

The Clinical Documentation Specialist facilitates modifications to clinical documentation through concurrent interaction with providers and other members of the healthcare team. Promoting capture of clinical severity to support the level of service rendered to patients. Clinically evaluating how the health record translates into coded data. Communicating with providers regarding missing, unclear or conflicting health record documentation. Facilitating provider education regarding documentation of disease processes to ensure accurate application of the complexity, acuity and severity of illness within the code sets.

Daily Tasks:

* Review assigned cases within Epic WQ's for initial review admission cases
* Follow up on subsequent reviews for cases initiated
* Follow up on outstanding queries, escalate as needed
* Complete final reviews to close cases once all items have been addressed
* Additional case reviews (Observation, other payers) available if main work is complete

Weekly Tasks:

* Communicate issues/delays to Hospitalist Lead Physician and CDI Manager
* Assist with workload for CDI reviews for co-worker(s), when needed
* Identify areas for education/improvement opportunities

Monthly Tasks:

* Create educational slides/materials for physician education related to CDI topics
* Attend appropriate meetings to address education with physicians

Quarterly Tasks:

* Participate in quarterly coding update education

Annual Tasks:

* Complete WebEd courses
* Participate in annual coding update education
* Complete CEUs if needed for Certification requirements

As Needed/Requested:

* Research of CDI topics
* Coding experience preferred
* Adhoc reporting on case reviews
