Active Ohio Physical Therapy license required prior to start of assignment. Current CPR certification required. Valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel within the service area. Experience providing Physical Therapy in home health settings preferred. Ability to complete patient evaluations, develop plan of care, and document per CMS and agency requirements. Willingness and ability to participate in weekend rotation and potential holiday availability as needed. This is required by the client.
