What You Should Know About the RN Care Transitions Coordinator (Admissions) Role:

  • This position will do the office work for our admissions team (certifications, SBARS, medication needs, etc) and will also be cross trained in admissions and will be asked to travel
  • Schedule will be full-time, 36 hours/week. 8A-8P with a weekend rotation
  • We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The RN Care Transitions Coordinator (Admissions) Essential Duties Are:

Works with patients, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care from acute care across the continuum. Ensures consistency and timely care transition, speed to care by minimizing redundancy in handoffs and provides consistency for patient/family.


  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
