Description As a Hospice Medical Social Worker, you will be called to care when you're needed most. As part of Interim HealthCare, you'll support a full range of patient services to bring comfort and dignity to our clients. As a Hospice Medical Social Worker, you will: Provide direct patient counseling services; support crisis interventions as needed Assist patients and family in community resource planning and access Help the patient and family navigate their ongoing healthcare journey To qualify as a Hospice Medical Social Worker with us, you will need: Education: Bachelor of Science in Social Work (BSW) or comparable undergraduate degree required; Master of Social Work (MSW) preferred Licensure: Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required; Licensed Independent Social Worker (LISW) preferred One (1) year of experience practicing as a Licensed Social Worker in a hospice/palliative care or similar setting Reliable transportation to/from care sites/ or work locations At Interim HealthCare Hospice & Palliative Care, we know that being our best is non-negotiable - that's why we treat your family like our own.
